Midlands EMS crews were dispatched to the N3 near Mooi River following a truck collision.
Upon arrival, it was found that the truck had gone off the road and rolled multiple times.
The driver sustained serious injuries and required urgent advanced life support treatment.
Once stabilized, the patient was transported to a Level 1 trauma hospital for further care.
Authorities are still on the scene.
